Patient Navigator, Cancer & Supportive Care
Cancer and Supportive Care Temporary Full-time Position (Approximately 1 Year)
St. Martha's Regional Hospital, Antigonish, Nova Scotia
Position Scope: Patient Navigation is the process of guiding cancer patients (children, adolescents, adults) and their families, through the cancer system. The Patient Navigator will be one of the first points of contact for physicians and patients once the patient is diagnosed with cancer.
The position reports to the District Manager of Cancer and Supportive Care. The Patient Navigator will be employed with the Guysborough Antigonish Strait Health Authority and will work in conjunction with the District Cancer Program to respond to requests from family physicians, community based specialists, health professionals, patients and families in order to facilitate cancer investigation, patient care and treatment services. The Patient Navigator provides information and education for patients and families and functions as a liaison between the healthcare setting and the community to refer individuals to community based support and rehabilitation services as required.
Qualifications:
- BScN with minimum of five years experience dealing with patients and families experiencing cancer.
- Experience in patient navigator preferred.
- A combination of equivalent education and experience may be considered
Requirements:
- Certification in oncology required or willingness to obtain certification.
- Knowledge of the provincial cancer system a requirement
- Demonstrated competence in conflict management
- Demonstrated competence in team building
- Demonstrated in-dept knowledge of clinical and supportive cancer care
- Demonstrated excellent communication and interpersonal skills
- Demonstrated excellent time management and organizational skills
- Able to work independently as well as a member of the health care team
- Self motivated individual who has demonstrated ability to show initiative and creativity
- Preference will be given to candidates with knowledge of the strengths and challenges within the district health authority.
- Travel throughout the district will be required.
- Must be able to work flexible hours.
- Work closely with Senior Management to take a leadership role in developing the Cancer Care Program within the District.
