Duties:

The Care Coordinator is responsible for intake, assessment, determination of financial eligibility and support needs, support planning, coordination and implementation of programs and services, ongoing case management, monitoring and discharge planning of clients with disabilities within the Services for Persons with Disabilities Program, which services the needs of children with intellectual and physical disabilities and adults with long term mental illness, intellectual and/or physical disability.

Qualifications:

Bachelor's Degree in either Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutritional Studies plus five years related experience, or a Diploma in Nursing plus nine years related experience.  Must be currently registered, or eligible for registration with applicable professional association.  An approved and acceptable combination of education and experience may be considered. Must have excellent assessment, interviewing and negotiation skills, which are essential in dealing with complex client situations.  The ideal candidate will be a team player who displays sound judgement and has well developed skills in critical thinking, problem solving and time management.  Candidates must have a solid knowledge base in child development and  thorough understanding of the needs and issues relating to children, adults and families with a person with a mental and/or physical disability, a good understanding of community supports available and the dynamics of facilitating the development of individual family support networks and family-centred service delivery.  Knowledge of health and social services systems, long term care and case management principles, risk management, community development approaches, coordination of appropriate services, special needs populations, Homes for Special Care legislation and Interim Guidelines for Community-based Options is required.  Must have access to a reliable vehicle to travel to various sites.

Classification Range:   HSN 27

Salary Range:     $29.62 - $33.97
